1. How should I take Norgesic®?
Take Norgesic® with the flexible dosing regime provided to you by your heath care provider.
Taking Norgesic®
For adults:
1 to 2 tablets
3 to 4 times a day.

2. What types of pain can Norgesic® help relieve?
Norgesic® can help to relieve pain caused by:
- Common strains and sprains
- Low back pain
- Muscle aches
- Headaches caused by neck/shoulder problems
- Joint pain, such as arthritis
- Musculoskeletal injuries or other painful conditions that affect muscles, ligaments, tendons, and bones
Norgesic® is used alongside rest, physical therapy, and other measures used to relieve pain due to these conditions.
3. What important information do I need to know while taking Norgesic®?
Norgesic® may make you drowsy. Do not drive, operate machinery, participate in dangerous activities, or do anything that requires you to be alert until you know how Norgesic® affects you.
If you take too much Norgesic® or overdose, seek emergency medical treatment right away.
4. Who should not take Norgesic®?
Do not use Norgesic® if you are pregnant. Children under the age of 12 should not take Norgesic®. Do not use Norgesic® if you have chicken pox, flu, or flu-like symptoms.
Aspirin, which is found in Norgesic®, can increase the risk of a serious and potentially fatal condition called Reye’s Syndrome in children and those who are suffering from chicken pox, flu, or flu-like symptoms.

What is the most important information I should know about Norgesic®?
Norgesic® tablets may cause serious side effects, including:
- Rare but serious bleeding due to the aspirin content
- Life-threatening skin reactions
- Life-threatening allergic reactions
Stop taking Norgesic® and call your healthcare provider or get emergency help right away if you have any of these symptoms: rash, fever, trouble breathing, or swelling of the face, lips, tongue, or throat.
Use caution until you know how Norgesic® affects you. It may impair your ability to drive or use other machinery.
Do not take Norgesic® if you have any of the following:
- Glaucoma (an eye disease)
- Blockage in your digestive tract (pyloric or duodenal obstruction)
- Achalasia (a rare disorder of the esophagus)
- Enlarged prostate (prostatic hypertrophy)
- Blockage that results in trouble urinating (obstruction of the bladder neck)
- Myasthenia gravis (a disorder of the muscles and nerves)
- Allergy or sensitivity to orphenadrine, aspirin, caffeine, or any other ingredient in Norgesic®

Before taking Norgesic®, tell your healthcare provider about all your medical conditions, including active viral infection (such as chicken pox, flu, or flu-like symptoms), stomach ulcer, bleeding problems, or pregnancy (or plans to become pregnant).
Talk to your healthcare provider if you are considering taking Norgesic® during pregnancy. You should not take Norgesic® after 29 weeks of pregnancy.
What are the possible side effects of Norgesic®?
Common side effects of Norgesic® include increased heart rate (tachycardia), strong or rapid heartbeat (palpitations), trouble urinating (urinary hesitancy or retention), dry mouth, blurred vision, weakness, nausea, vomiting, headache, dizziness, constipation, or drowsiness.

ACTIONS
Orphenadrine citrate is a centrally acting (brain stem) compound which in animals selectively blocks facilitatory functions of the reticular formation. Orphenadrine does not produce myoneural block, nor does it affect crossed extensor reflexes. Orphenadrine prevents nicotine-induced convulsions but not those produced by strychnine.
Chronic administration of Norgesic to dogs and rats has revealed no drug-related toxicity. No blood or urine changes were observed, nor were there any macroscopic or microscopic pathological changes detected. Extensive experience with combinations containing aspirin and caffeine has established them as safe agents. The addition of orphenadrine citrate does not alter the toxicity of aspirin and caffeine.
The mode of therapeutic action of orphenadrine has not been clearly identified, but may be related to its analgesic properties. Orphenadrine citrate also possesses anticholinergic actions.
INDICATIONS
- Symptomatic relief of mild to moderate pain of acute musculoskeletal disorders.
- The orphenadrine component is indicated as an adjunct to rest, physical therapy, and other measures for the relief of discomfort associated with acute painful musculoskeletal conditions.
The mode of action of orphenadrine has not been clearly identified, but may be related to its analgesic properties. Norgesic and Norgesic Forte do not directly relax tense skeletal muscles in man.
NORGESIC CONTRAINDICATIONS
Because of the mild anticholinergic effect of orphenadrine, Norgesic or Norgesic Forte should not be used in patients with glaucoma, pyloric or duodenal obstruction, achalasia, prostatic hypertrophy or obstructions at the bladder neck. Norgesic or Norgesic Forte is also contraindicated in patients with myasthenia gravis and in patients known to be sensitive to aspirin or caffeine.
The drug is contraindicated in patients who have demonstrated a previous hypersensitivity to the drug.
WARNINGS
Reye’s Syndrome may develop in individuals who have chicken pox, influenza, or flu symptoms. Some studies suggest a possible association between the development of Reye’s Syndrome and the use of medicines containing salicylate or aspirin. Norgesic and Norgesic Forte contain aspirin and therefore are not recommended for use in patients with chicken pox, influenza, or flu symptoms.
Norgesic and Norgesic Forte may impair the ability of the patient to engage in potentially hazardous activities such as operating machinery or driving a motor vehicle; ambulatory patients should therefore be cautioned accordingly.
Aspirin should be used with extreme caution in the presence of peptic ulcers and coagulation abnormalities.
USAGE IN PREGNANCY
Since safety of the use of this preparation in pregnancy, during lactation, or in the childbearing age has not been established, use of the drug in such patients requires that the potential benefits of the drug be weighed against its possible hazard to the mother and child.
PEDIATRIC USE
Safety and effectiveness in pediatric patients have not been established.
PRECAUTIONS
Confusion, anxiety and tremors have been reported in a few patients receiving propoxyphene and orphenadrine concomitantly. As these symptoms may be simply due to an additive effect, reduction of dosage and/or discontinuation of one or both agents is recommended in such cases.
Safety of continuous long term therapy with Norgesic and Norgesic Forte has not been established; therefore, if Norgesic or Norgesic Forte is prescribed for prolonged use, periodic monitoring of blood, urine and liver function values is recommended.
NORGESIC ADVERSE REACTIONS
Side effects of Norgesic or Norgesic Forte are those seen with aspirin and caffeine or those usually associated with mild anticholinergic agents. These may include tachycardia, palpitation, urinary hesitancy or retention, dry mouth, blurred vision, dilatation of the pupil, increased intraocular tension, weakness, nausea, vomiting, headache, dizziness, constipation, drowsiness, and rarely, urticaria and other dermatoses. Infrequently, an elderly patient may experience some degree of confusion. Mild central excitation and occasional hallucinations may be observed. These mild side effects can usually be eliminated by reduction in dosage. One case of aplastic anemia associated with the use of Norgesic has been reported. No causal relationship has been established. Rare G.I. hemorrhage due to aspirin content may be associated with the administration of Norgesic or Norgesic Forte. Some patients may experience transient episodes of light-headedness, dizziness or syncope.
NORGESIC DOSAGE AND ADMINISTRATION
Norgesic: Adults 1 to 2 tablets 3 to 4 times daily.
Norgesic Forte: Adults ½ to 1 tablet 3 to 4 times daily.
HOW SUPPLIED
Norgesic tablets can be identified by their two layers colored white and yellow. Each round tablet is embossed “NORGESIC” on one side and “3M” on the other and contains orphenadrine citrate (2-dimethylaminoethyl 2-methylbenzhydryl ether citrate) 25 mg, aspirin 385 mg, and caffeine 30 mg.
Norgesic Forte tablets are exactly twice the strength of Norgesic. They are identified by their scored capsule shape and by their two layers colored white and yellow. Each capsule shaped tablet is embossed “NORGESIC FORTE” on one side and “3M” on the other and contains orphenadrine citrate 50 mg, aspirin 770 mg, and caffeine 60 mg.
Norgesic and Norgesic Forte also contain: lactose, polyethylene glycol, povidone, starch, sucrose, zinc stearate, and D&C yellow #10.

What Is Norgesic?
Norgesic (orphenadrine citrate, aspirin and caffeine) is a combination of a muscle relaxant, a pain reliever, and caffeine which increases the pain relieving effects of the other drugs, used, along with rest and physical therapy, to treat injuries and other painful muscular conditions. Norgesic is available in generic form.
What Are Side Effects of Norgesic?
Common side effects of Norgesic include dizziness, drowsiness, lightheadedness, blurred vision or large pupils, weakness, headache, stomach upset, heartburn, nausea, vomiting, constipation, dry mouth, tremor, difficulty urinating or increased urination, hiccups, trouble sleeping, or ringing in the ears.
Dosage for Norgesic
The adult dose of Norgesic is 1 to 2 tablets, 3 to 4 times daily. The adult dose of Norgesic Forte is ½ to 1 tablet 3 to 4 times daily.
What Drugs, Substances, or Supplements Interact with Norgesic?
Norgesic may interact with antidepressants, alcohol, antihistamines, sedatives, other pain relievers, anxiety medicines, muscle relaxants, anticoagulants, nonsteroidal anti-inflammatory drugs (NSAIDs), or other salicylates. Tell your doctor all medications you use.
Norgesic During Pregnancy and Breastfeeding
During pregnancy, Norgesic should be used only when prescribed. Aspirin is not recommended for use during pregnancy. This medication passes into breast milk and may harm a nursing infant. Breastfeeding while using this drug is not recommended.

Fetal Toxicity
Premature Closure Of Fetal Ductus Arteriosus
Avoid use of NSAIDs, including Norgesic and Norgesic Forte, in pregnant women at about 30 weeks gestation and later. NSAIDs including Norgesic and Norgesic Forte, increase the risk of premature closure of the fetal ductus arteriosus at approximately this gestational age.
Oligohydramnios/Neonatal Renal Impairment
Use of NSAIDs, including Norgesic and Norgesic Forte, at about 20 weeks gestation or later in pregnancy may cause fetal renal dysfunction leading to oligohydramnios and, in some cases, neonatal renal impairment. These adverse outcomes are seen, on average, after days to weeks of treatment, although oligohydramnios has been infrequently reported as soon as 48 hours after NSAID initiation.
Oligohydramnios is often, but not always, reversible with treatment discontinuation. Complications of prolonged oligohydramnios may, for example, include limb contractures and delayed lung maturation. In some postmarketing cases of impaired neonatal renal function, invasive procedures such as exchange transfusion or dialysis were required.
If NSAID treatment is necessary between about 20 weeks and 30 weeks gestation, limit Norgesic and Norgesic Forte use to the lowest effective dose and shortest duration possible. Consider ultrasound monitoring of amniotic fluid if Norgesic or Norgesic Forte treatment extends beyond 48 hours. Discontinue Norgesic or Norgesic Forte if oligohydramnios occurs and follow up according to clinical practice [see PRECAUTIONS, Pregnancy].
Drug Reaction With Eosinophilia And Systemic Symptoms (DRESS)
Drug Reaction with Eosinophilia and Systemic Symptoms (DRESS) has been reported in patients taking NSAIDs such as Norgesic and Norgesic Forte. Some of these events have been fatal or life-threatening. DRESS typically, although not exclusively, presents with fever, rash, lymphadenopathy, and/or facial swelling.
Other clinical manifestations may include hepatitis, nephritis, hematological abnormalities, myocarditis, or myositis. Sometimes symptoms of DRESS may resemble an acute viral infection. Eosinophilia is often present. Because this disorder is variable in its presentation, other organ systems not noted here may be involved. It is important to note that early manifestations of hypersensitivity, such as fever or lymphadenopathy, may be present even though rash is not evident. If such signs or symptoms are present, discontinue Norgesic or Norgesic Forte and evaluate the patient immediately.
PRECAUTIONS

Pregnancy
Risk Summary
Use of NSAIDs, including aspirin, can cause premature closure of the fetal ductus arteriosus and fetal renal dysfunction leading to oligohydramnios and, in some cases, neonatal renal impairment. Because of these risks, limit dose and duration of Norgesic and Norgesic Forte use between about 20 and 30 weeks of gestation, and avoid Norgesic and Norgesic Forte use at about 30 weeks of gestation and later in pregnancy [see WARNINGS; Fetal Toxicity].
Premature Closure Of Fetal Ductus Arteriosus
Use of NSAIDs, including aspirin, at about 30 weeks gestation or later in pregnancy increases the risk of premature closure of the fetal ductus arteriosus.
Oligohydramnios/Neonatal Renal Impairment
Use of NSAIDs at about 20 weeks gestation or later in pregnancy has been associated with cases of fetal renal dysfunction leading to oligohydramnios, and in some cases, neonatal renal impairment.
Data from observational studies regarding other potential embryofetal risks of NSAID use in women in the first or second trimesters of pregnancy are inconclusive. In the general U.S. population, all clinically recognized pregnancies, regardless of drug exposure, have a background rate of 2-4% for major malformations, and 15-20% for pregnancy loss.
Based on animal data, prostaglandins have been shown to have an important role in endometrial vascular permeability, blastocyst implantation, and decidualization. In animal studies, administration of prostaglandin synthesis inhibitors such as aspirin, resulted in increased pre-and post-implantation loss. Prostaglandins also have been shown to have an important role in fetal kidney development. In published animal studies, prostaglandin synthesis inhibitors have been reported to impair kidney development when administered at clinically relevant doses.
Clinical Considerations
Fetal/Neonatal Adverse Reactions
Premature Closure of Fetal Ductus Arteriosus
Avoid use of NSAIDs in women at about 30 weeks gestation and later in pregnancy, because NSAIDs, including Norgesic and Norgesic Forte, can cause premature closure of the fetal ductus arteriosus [see WARNINGS; Fetal Toxicity].
Oligohydramnios/Neonatal Renal Impairment
If an NSAID is necessary at about 20 weeks gestation or later in pregnancy, limit the use to the lowest effective dose and shortest duration possible. If Norgesic or Norgesic Forte treatment extends beyond 48 hours, consider monitoring with ultrasound for oligohydramnios. If oligohydramnios occurs, discontinue Norgesic or Norgesic Forte and follow up according to clinical practice [see WARNINGS; Fetal Toxicity].

Premature Closure Of Fetal Ductus Arteriosus
Published literature reports that the use of NSAIDs at about 30 weeks of gestation and later in pregnancy may cause premature closure of the fetal ductus arteriosus.
Oligohydramnios/Neonatal Renal Impairment
Published studies and postmarketing reports describe maternal NSAID use at about 20 weeks gestation or later in pregnancy associated with fetal renal dysfunction leading to oligohydramnios, and in some cases, neonatal renal impairment. These adverse outcomes are seen, on average, after days to weeks of treatment, although oligohydramnios has been infrequently reported as soon as 48 hours after NSAID initiation. In many cases, but not all, the decrease in amniotic fluid was transient and reversible with cessation of the drug. There have been a limited number of case reports of maternal NSAID use and neonatal renal dysfunction without oligohydramnios, some of which were irreversible. Some cases of neonatal renal dysfunction required treatment with invasive procedures, such as exchange transfusion or dialysis.
Methodological limitations of these postmarketing studies and reports include lack of a control group; limited information regarding dose, duration, and timing of drug exposure; and concomitant use of other medications. These limitations preclude establishing a reliable estimate of the risk of adverse fetal and neonatal outcomes with maternal NSAID use. Because the published safety data on neonatal outcomes involved mostly preterm infants, the generalizability of certain reported risks to the full-term infant exposed to NSAIDs through maternal use is uncertain.

CONTRAINDICATIONS
Because of the mild anti-cholinergic effect of orphenadrine, Norgesic and Norgesic Forte Tablets should not be used in patients with glaucoma, pyloric or duodenal obstruction, achalasia, prostatic hypertrophy, or obstructions at the bladder neck. Norgesic and Norgesic Forte Tablets are also contraindicated in patients with myasthenia gravis and in patients known to be sensitive to aspirin or caffeine.
The drug is contraindicated in patients who have demonstrated a previous hypersensitivity to the drug.
CLINICAL PHARMACOLOGY
Orphenadrine citrate is a centrally acting (brain stem) compound which in animals selectively blocks facilitatory functions of the reticular formulation. Orphenadrine does not produce myoneural block, nor does it affect crossed extensor reflexes. Orphenadrine prevents nicotine-induced convulsions but not those produced by strychnine.
Chronic administration of Orphenadrine Citrate, Aspirin, and Caffeine to dogs and rats has revealed no drug-related toxicity. No blood or urine changes were observed, nor were there any macroscopic or microscopic pathological changes detected. Extensive experience with combinations containing aspirin and caffeine has established them as safe agents. The addition of orphenadrine citrate does not alter the toxicity of aspirin and caffeine.
The mode of therapeutic action of orphenadrine has not been clearly identified, but may be related to its analgesic properties. Orphenadrine citrate also possesses anticholinergic actions.

Pregnancy
Embryo-Fetal Toxicity
Inform pregnant women to avoid use of aspirin and other NSAIDs starting at 30 weeks gestation because of the risk of the premature closing of the fetal ductus arteriosus. If treatment with Norgesic or Norgesic Forte is needed for a pregnant woman between about 20 to 30 weeks gestation, advise her that she may need to be monitored for oligohydramnios, if treatment continues for longer than 48 hours [see WARNINGS, Fetal Toxicity; PRECAUTIONS, Pregnancy].
Serious Skin Reactions, Including DRESS
Advise patients to stop taking Norgesic or Norgesic Forte immediately if they develop any type of rash or fever and to contact their healthcare provider as soon as possible [see WARNINGS].

Why am I using NORGESIC?
NORGESIC contains the active ingredients orphenadrine citrate and paracetamol. NORGESIC
is used to treat tension headache and headaches caused by muscle spasms in the back
of your head and neck. It also helps relax certain muscles in your body and to relieve
the pain and discomfort caused by sprains, strains or other injury to your muscles.

Do not use if you have ever had an allergic reaction to NORGESIC or any of the ingredients
listed at the end of the CMI.

Side effects include nausea, dry mouth, blurred vision, drowsiness, rash, trouble
urinating, fainting, fast or pounding heartbeat, hallucinations, trouble breathing,
unusual bruising or bleeding, confusion and constipation.

Why am I using NORGESIC?
NORGESIC contains the active ingredients orphenadrine citrate and paracetamol. Orphenadrine citrate is a skeletal muscle relaxant. It acts in the central nervous
system to produce muscle relaxant effects. Paracetamol is used to treat or prevent
pain and reduce fever.
NORGESIC is used to treat tension headache and headaches caused by spasm of the muscles
in the back of your head and neck.
It is also used to help relax certain muscles in your body and to relieve the pain
and discomfort caused by sprains, strains or other injury to your muscles.
What should I know before I use NORGESIC?
Warnings
Do not use NORGESIC if:
You are allergic to orphenadrine citrate, paracetamol or any of the ingredients listed
at the end of this leaflet. Always check the ingredients to make sure you can use
this medicine.
You have any of the following conditions:
Glaucoma
Intestinal blockage or oesophageal disease
An enlarged prostate or bladder obstruction
Myasthenia gravis (muscle weakness)
The packaging shows signs of tampering or is after the expiry date.
Check with your doctor if you:
have any heart, liver, or kidney problems
have any alcohol abuse problems
have any other medical conditions
are taking other medicines containing paracetamol
take any medicines for any other condition.
During treatment, you may be at risk of developing certain side effects. It is important
you understand these risks and how to monitor for them. See additional information
under Section 6. Are there any side effects?
Pregnancy and breastfeeding
Check with your doctor if you are pregnant or intend to become pregnant.
Talk to your doctor if you are breastfeeding or intend to breastfeed.
Children under 12 years old
Do not give NORGESIC to children under the age of 12 years.
What if I am taking other medicines?
Tell your doctor or pharmacist if you are taking any other medicines, including any
medicines, vitamins or supplements that you buy without a prescription from your pharmacy,
supermarket or health food shop.
Some medicines may interfere with NORGESIC and affect how it works including:
Phenothiazine medicines used to treat serious emotional and mental disorders
Medicines used to relieve stomach cramps or spasms
Medicines which have a sedating effect
Alcohol
Medicines to prevent blood clots
Medicines that affect how quickly food leaves the stomach, such as metoclopramide,
propantheline, some antidepressants and narcotic painkillers
Chloramphenicol, an antibiotic.
Check with your doctor or pharmacist if you are not sure about what medicines, vitamins
or supplements you are taking and if these affect NORGESIC.
How do I use NORGESIC?
How much to take
The usual dose is two tablets three times a day.
Do not take more than the recommended dose unless your doctor tells you to.
Follow the instructions provided and use NORGESIC until your doctor tells you to stop.
When to take NORGESIC
NORGESIC should be used as directed by your doctor or pharmacist.
Do not change the dose unless your doctor tells you to do so.
If you forget to take NORGESIC
NORGESIC should be used regularly at the same time each day. If you miss your dose
at the usual time and remember within an hour or so of the missed dose, take it straight
away.
If it is almost time for your next dose, skip the dose you missed and take your next
dose when you are meant to.
Do not take a double dose to make up for the dose you missed.
If you use too much NORGESIC
If you think that you have used too much NORGESIC, you may need urgent medical attention.
Overdose with paracetamol-containing medicines can lead to liver damage.
You should immediately:
phone the Poisons Information Centre
(by calling
13 11 26), or
contact your doctor, or
go to the Emergency Department at your nearest hospital.
Do this even if there are no signs of discomfort or poisoning.
What should I know while using NORGESIC?
Things you should do
Call your doctor straight away if you:
become pregnant
Remind any doctor, dentist or pharmacist you visit that you are using NORGESIC.
NORGESIC may cause dryness of the mouth. For temporary relief use sugarless gum or
sweets, melt some ice in your mouth or use a saliva substitute. If dry mouth persists,
talk to your dentist.
Things you should not do
Do not give NORGESIC to anyone else even if they have the same condition as you.
Do not take NORGESIC to treat any other complaints unless your doctor tells you to.
Driving or using machines
Be careful before you drive or use any machines or tools until you know how NORGESIC
affects you.
NORGESIC may cause blurred vision or drowsiness in some people.
Drinking alcohol
Tell your doctor if you drink alcohol.
Drinking alcohol whilst using NORGESIC may increase the occurrence or severity of
side effects such as drowsiness.
Looking after your medicine
Follow the instructions on the carton on how to take care of your medicine properly.
Store NORGESIC below 30°C in a cool dry place away from moisture, heat or sunlight;
for example, do not store it:
in the bathroom or near a sink, or
in the car or on windowsills.
Keep it where young children cannot reach it.
Getting rid of any unwanted medicine
If you no longer need to use this medicine or it is out of date, take it to any pharmacy
for safe disposal.
Do not use this medicine after the expiry date.
Are there any side effects?
All medicines can have side effects. If you do experience any side effects, most of
them are minor and temporary. However, some side effects may need medical attention.
