Антиангинальное и антиишемическое средство.

Механизм действия

Благодаря сохранению энергетического метаболизма в клетках, которые страдают от гипоксии или ишемии, триметазидин предотвращает уменьшение уровня внутриклеточного АТФ, обеспечивая тем самым надлежащее функционирование ионных насосов и трансмембранного натриево-калиевого потока при сохранении клеточного гомеостаза.

Триметазидин тормозит β-окисление жирных кислот, блокируя длинноцепочечную 3-кетоацил- КоА-тиолазу (3-КАТ), что повышает окисление глюкозы. В клетках в условиях ишемии процесс получения энергии путем окисления глюкозы требует меньше кислорода сравнительно с процессом получения энергии путем β-окисления жирных кислот. Усиление процесса окисления глюкозы оптимизирует энергетические процессы в клетках и соответственно поддерживает достаточный метаболизм энергии в условиях ишемии.

Одновременно триметазидин увеличивает обмен фосфолипидов и их включение в мембрану, обеспечивая тем самым защиту мембраны от повреждений.

При стенокардии сокращает частоту приступов, уменьшает потребность в применении нитроглицерина, повышает толерантность к физической нагрузке, не влияет на артериальное давление и частоту сердечных сокращений.

Фармакодинамические эффекты

У пациентов с ишемической болезнью сердца триметазидин действует как метаболический агент, сохраняя внутриклеточные уровни высокоэнергетических фосфатов в миокарде. Эффекты достигаются без сопутствующих гемодинамических эффектов.

Фармакокинетика

После приема внутрь быстро и практически полностью абсорбируется слизистой оболочкой кишечника; прием пищи не влияет на всасывание препарата. Максимальная концентрация в плазме крови достигается через 2-3 часа и прямо пропорционально зависит от дозы. Связывание с белками плазмы крови незначительное – 16-21 %. Триметазидин биотрансформируется с образованием трех основных и нескольких второстепенных метаболитов. Хорошо распределяется в тканях, объем распределения – около 4,8 л/кг. Период полувыведения составляет 6-7 часов. Экскретируется преимущественно почками (85 %), незначительное количество – кишечником (6 %). Данные о проникновении триметазидина в грудное молоко отсутствуют.

Показания

Взрослым препарат показан для симптоматического лечения стабильной стенокардии при условии недостаточной эффективности или непереносимости антиангинальных препаратов первой линии.

Повышенная чувствительность к действующему веществу или к любому вспомогательному веществу, болезнь Паркинсона, симптомы паркинсонизма, тремор, синдром «беспокойных ног» и другие связанные с ним двигательные расстройства, тяжелая почечная недостаточность (клиренс креатинина < 30 мл/мин).

Взаимодействие с лекарственными средствами

Случаев взаимодействия с другими лекарственными средствами отмечено не было.

Если Вы принимаете любые другие лекарственные средства, проконсультируйтесь с врачом относительно возможности применения препарата.

Препарат применяется для базисной терапии стенокардии, но не для купирования приступов стенокардии. Его не следует назначать при нестабильной стенокардии или инфаркте миокарда в качестве первичной терапии на догоспитальном этапе или в первые дни госпитализации. При возникновении приступа нестабильной стенокардии на фоне текущей терапии необходимо пересмотреть лечение и назначить соответствующую медикаментозную терапию и, возможно, реваскуляризацию, учитывая тяжесть заболевания.

Триметазидин может вызывать или ухудшать симптомы паркинсонизма (тремор, акинезия, гипертонус мышц), которые следует регулярно исследовать, особенно у пациентов пожилого возраста. В сомнительных случаях пациентов нужно направлять к невропатологу для соответствующих исследований. При появлении двигательных расстройств, таких как симптомы паркинсонизма, синдрома «беспокойных ног», тремора, нестойкости походки, необходимо отменить триметазидин. Данные случаи имеют низкую частоту и обычно исчезают после прекращения лечения; у большинства пациентов – в течение 4 месяцев после прекращения приема триметазидина. Если симптомы паркинсонизма сохраняются более 4 месяцев после отмены препарата, необходимо обратиться к невропатологу.

Могут быть падения, связанные с нестойкостью походки или артериальной гипотензией, особенно у пациентов, которые проводят антигипертензивное лечение (см. раздел «Побочное действие»).

Необходимо с осторожностью назначать триметазидин:

пациентам с умеренной почечной недостаточностью;

пациентам в возрасте свыше 75 лет.

При почечной недостаточности рекомендуется проводить мониторинг функциональных показателей, при необходимости дозу можно снизить.

Применение препарата не требует особых предостережений перед проведением анестезии.

Применение в период беременности или кормления грудью

В связи с отсутствием клинических данных применение препарата не рекомендуется в период беременности или кормления грудью.

Способность влиять на скорость реакции при управлении автотранспортом или другими механизмами

Триметазидин не влияет на гемодинамику, однако были зафиксированы случаи головокружения и сонливости (см. раздел «Побочное действие»), которые могут влиять на способность управлять автомобилем и работать с механизмами.

Дети

Безопасность и эффективность триметазидина для детей (в возрасте до 18 лет) не установлены. Данные отсутствуют.

Препарат назначать взрослым по 1 таблетке 3 раза в сутки во время еды, запивая достаточным количеством воды. Длительность терапии устанавливается индивидуально, в зависимости от тяжести и течения заболевания.

Пациенты с почечной недостаточностью

Для пациентов с умеренным нарушением функции почек (клиренс креатинина 30-60 мл/мин) (см. раздел «Меры предосторожности») рекомендованная доза составляет 1 таблетку 2 раза в сутки, т.е. утром и вечером во время еды.

Пациенты пожилого возраста

Пациенты пожилого возраста более чувствительны к действию триметазидина из-за возрастного снижения функции почек. Для пациентов с умеренным нарушением функции почек (клиренс креатинина 30-60 мл/мин) рекомендованная доза составляет 1 таблетку 2 раза в сутки, т.е. утром и вечером во время еды. Для пациентов пожилого возраста необходимо внимательно титровать дозу.

В случае, если прием препарата был пропущен, необходимо принять следующую дозу в установленное время. Не следует принимать двойную дозу с целью восполнения пропущенной.

Широкий терапевтический диапазон делает маловероятными случаи серьезной интоксикации. В случае передозировки (приема значительного количества препарата) может возникнуть снижение периферического сопротивления сосудов и, как следствие, возникновение артериальной гипотензии и покраснение лица. В таких случаях показано проведение симптоматической терапии.

Со стороны системы пищеварения: боль в эпигастральной области живота, диарея, диспепсия, тошнота и рвота, запор.

Общие нарушения: астения.

Со стороны нервной системы: головная боль, головокружение, экстрапирамидные симптомы (тремор, акинезия, нестойкость походки, гипертонус мышц, синдром «беспокойных ног» и другие двигательные расстройства, имеющие отношение к вышеупомянутому), которые обычно проходят после отмены лечения, нарушения сна (бессонница, сонливость).

Со стороны кожи и подкожных тканей: сыпь, зуд, крапивница, эритема, острая генерализованная экзантематозная пустулезная сыпь, ангионевротический отек.

Со стороны сердечно-сосудистой системы: артериальная гипотензия, ортостатическая гипотензия (которая может быть ассоциирована с недомоганием, головокружением или падением, в частности у пациентов, применяющих гипертензивные средства), покраснение лица, пальпитация, экстрасистолия, тахикардия.

Со стороны системы крови и лимфатической системы: агранулоцитоз, тромбоцитопения, тромбоцитопеническая пурпура.

Со стороны гепатобилиарной системы: гепатит.

Из-за наличия в составе препарата красителей желтый закат FCF (Е 110) и Понсо 4R (Е 124) возможно возникновение аллергических реакций, включая астму, особенно у больных с аллергией на ацетилсалициловую кислоту.

При появлении указанных или любых нежелательных симптомов прием препарата следует прекратить и обратиться к врачу.

Препарат нельзя применять после окончания срока годности, указанного на упаковке.

Хранить в оригинальной упаковке при температуре не выше 25 °С.

Triguard is an anti-ischemic (anti-anginal) metabolic agent, which improves myocardial glucose utilization through inhibition of long-chain 3-ketoacyl CoA thiolase activity, which results in a reduction in fatty acid oxidation and a stimulation of glucose oxidation. High fatty acid oxidation rates are detrimental during ischemia due to an inhibition of glucose oxidation leading to uncoupling of glycolysis and an increase in proton production, which has the potential to accelerate sodium and calcium overload in the heart. This leads to an exacerbation of ischemic injury and decreased cardiac efficiency during reperfusion.

Triguard is indicated for the symptomatic treatment of stable angina pectoris in patients inadequately controlled or intolerant to first line therapies. Patients should be counselled regarding the risk of use with reduced renal or hepatic function, worsening of extrapyramidal symptoms or other movement disorders, and risk of falls.

How Triguard works

During myocardial ischemia, anaerobic metabolism takes over, increasing levels of lactic acid. The decreased intracellular pH and increased concentration of protons activates sodium-hydrogen and sodium-calcium antiport systems, raising intracellular calcium concentrations, finally leading to decreased contractility.

This injury to the myocardium raises concentrations of catecholamines, which activate hormone sensitive lipase, and increasing fatty acid concentrations in plasma. When the myocardium is repurfused, fatty acid oxidation becomes the dominant form of ATP production, maintaining an acidic pH, and further exacerbating the injury.

The mechanism of action of trimetazidine is not fully understood. Triguard may inhibit mitochondrial 3-ketoacyl coenzyme A thiolase, decreasing long chain fatty acid β-oxidation but not glycolysis in the myocardium. The decreased long chain fatty acid β-oxidation is compensated for by increased use of glucose, preventing a lowered myocardial pH, and further decreases in contractility. However, another study suggests that 3-ketoacyl coenzyme A thiolase may not be trimetazidine’s target, and that this mechanism may be incorrect.

Triguard dosage

One 20 mg tablet thrice daily after meals. No dosage adjustments are required in patients with impaired renal and hepatic function.

Side Effects

Triguard is safe and well tolerated. The most commonly encountered side effects are gastric discomfort, nausea, headache and vertigo. However, the side effects are mild and non-specific.

Toxicity

Data regarding overdoses of trimetazidine are not readily available. Treat overdoses with symptomatic and supportive therapy.

The oral LD₅₀ in rats is 1700 mg/kg, and in mice is 1550 mg/kg. The subcutaneous LD₅₀ in rats is 1500 mg/kg, and in mice is 410 mg/kg.

Interaction

No drug interactions have so far been reported. In particular, no interactions of Triguard with beta-blockers, calcium antagonists, nitrates, heparin, hypolipidemic agents or digitalis have been reported.

The volume of distribution of trimetazidine is 4.8 L/kg.

Elimination Route

In elderly patients, a 35 mg oral modified release tablet reaches a mean C_max of 115 µg/L, with a T_max of 2.0-5.0 hours, and a mean AUC_0-12 of 1104 h*µg/L. In young, healthy patients, the same dose reaches a mean C_max of 91.2 µg/L, with a T_max of 2.0-6.0 hours, and an AUC_0-12h 720 h*µg/L.

Half Life

In young, healthy subjects, the half life of trimetazidine is 7.81 hours. In patients over 65, the half life increases to 11.7 hours.

Clearance

Triguard clearance is strongly correlated with creatinine clearance. In eldery patients with a creatinine clearance of 72 ± 8 mL/min, trimetazidine clearance was 15.69 L/h. In young, healthy patients with a creatinine clearance of 134 ± 18 mL/min, trimetazidine clearance was 25.2 L/h.

Elimination Route

Triguard is 79-84% eliminated in the urine, with 60% as the unchanged parent compound. In a study of 4 healthy subjects, individual metabolites made up 0.01-1.4% of the dose recovered in urine.
